This type of request is a three step process. Please complete as many fields in the form below as possible, required fields are noted. In the second step you will be asked for your date of birth and social security number. In the third step you will be asked to print the generated form and mail or fax it to our office. Or, if you prefer, you can scan and email it when contacted by a caseworker from Congressman Moolenaar's office.
